Part 1: Purpose
The main purpose here is to demonstrate complex technique of a capsulorhexis radial tear out rescue with the use of cystotome in a human eye and on a cataract surgery simulator (virtual reality).
Part 2: Method
Here is a list of steps to be applied immediately after the initiation of a capsulorhexis to veer radially to the periphery beyond the pupillary margin:
Step 1 – Immediately stop capsulorhexis manoeuvre. Also re-examine the whole situation.
Step 2 – The anterior chamber should be filled with OVD (ophthalmic viscosurgical device).
Step 3 – Mount cystotome to a syringe filled with OVD. This helps reinflation of anterior chamber.
Step 4 – It is now time to leafe the capsulorhexis flap unfolded on the surface of lens.
Step 5 – Tilt tip of cystotome horizontally. This will avod puncturing or cutting of the flap. It will be engaged on the flap.
Step 6 – Look at the leading edge of tear and push or pull it right opposite to its direction.
Step 7 – The above step will make leading tearing edge do a ‘U-Turn’. The surgeon should maintain tension on the flap. This should be done until the return of tearing to the desired trajectory.
Part 3: Results
Use of this technique will help a surgeon respond immediately to radial tear out sans changing important surgical instruments. Switch of instruments at this point may be a risky affair. It can tear radial through sudden anterior chamber shallowing due to forward pressure ccoming from the vitreous. This technique ensures minimizing corneal wound distortion in conjunction with following collapse of anterior chamber.
Part 4: Discussion
The EYESI Surgical Simulator serves as an effective and realistic training platform for surgeons (novices and experienced). It helps them practice varied complex techniques of capsulorhexis tear-out. The most significant part of implantation process of endocapsular intraocular lens and phacoemulsification. It is also the most complex part of the process. Success of any cataract surgery depends on achievment of efficient capsulorhexis.
Surgeons may face a tough time while uundertaking the capsulorhexis. A common challenge is a radial tear-out. Hence, it is crucial for surgeons to gain detailed knowledge and acquaintance about tackling the problem promptly sans making the condition worse. Although there are many ways of rescuing situation through the use of capsulorhexis forceps, using an EYESi surgical simulator is the quickest and more effective.
